The Community Council is pleased to announce the re-launch of our Community Patient Transport Scheme, a vital service designed to support local residents in accessing hospital visits and other medical appointments.
This scheme helps reduce travel costs for patients and ensures that no one in our community is left without access to the care they need.
As part of the re-launch, we have introduced a newly designed application form to make the process more straightforward. Completed forms can now be downloaded and submitted directly to our dedicated email address: pts@pinwherryandpinmore.org.uk.
